BRISTOL, Ind. (AP) — A state agency says a northern Indiana company was partly responsible for the death of a 43-year-old employee who was killed when welding equipment fell from a forklift.

The investigation by Indiana's workplace safety agency revealed that Bravo Trailers in Bristol didn't ensure that each forklift operator was properly trained and ensure that forklifts only handled safely arranged loads.

Shawn Gray died on June 7. State regulators have proposed a $21,000 fine.

BRISTOL, Ind. (AP) — A man with a large blended family died in a workplace accident in Bristol when machinery being moved by a forklift fell onto him.

Shawn Gray, 43, died following the June 7 accident at Bravo Trailers in Bristol. Elkhart County's coroner says the Goshen man's preliminary cause of death was blunt injury trauma.

Gray's wife says her husband died after machinery fell onto him as it was being moved by a forklift at Bravo Trailers, which makes enclosed cargo trailers.

Angela Gray says she and her husband had a blended family that included his three daughters and her son and two daughters.

She says the family is devastated and awaiting answers about what caused the accident that took his life.
